This test will help you find out if you are at risk of developing a sexually transmitted disease (STD).

If your answer to any of the questions is "yes", then you may need to consult a doctor of the appropriate profile to learn more about your risk and find out if you need to be tested for a sexually transmitted infection.

The purpose of this test is not to diagnose sexually transmitted diseases. It also cannot predict if you will become infected in the future. The test will help you determine if you are at risk of developing a sexually transmitted disease.


The questionnaire is an analogue of the CDC (Center for Disease Control and Prevention) questionnaire.

Have you had vaginal, anal or oral sex without a condom in the last 12 months? *

Have you ever had a sexually transmitted disease? *

Have any of your sexual partners had a sexually transmitted infection?*

Have you had sex because of needs (money, housing, drugs, etc.) in the last 12 months?*

Is it possible that during the last 12 months, either of your sexual partners had sexual intercourse with another while they were still having sex with you?*

Have you or any of your partners used illegal drugs?*
