General Information

Thyroid Stimulating Hormone (TSH) Is a glycoprotein produced in basophilic cells of the anterior pituitary gland. TSH is released by a negative feedback mechanism in response to changes in the concentration of free triiodothyronine and free thyroxine. It consists of two chains - α and β. The α chain is common for TSH, follicle-stimulating hormone (FSH), melatoninizing hormone (LH), and chorionic gonadotropin-releasing hormone (hCG). The beta chain is different and determines its biochemical specificity. TSH has a circadian rhythm of secretion controlled by hypothalamic-releasing TSH-releasing hormone (TRH).

TSH interacts with specific receptors on the surface of thyroid cells and causes two main effects: cell reproduction and hypertrophy and stimulation of the synthesis and secretion of triiodothyronine and thyroxine by the thyroid gland.

Determination of TSH is the initial test in the diagnostics of thyroid disorders. Even a small change in the concentration of thyroid hormones causes a change in the concentration of TSH. If there are obvious signs of hypothyroidism but TSH levels are not elevated, secondary hypothyroidism during hypopituitarism is unlikely. Decreased levels of TSH are found in hyperthyroidism. Determination of TSH is useful for monitoring treatment in patients with hypothyroidism: low values ​​are found in case of overdose of thyroid hormone replacement drugs, while normal values ​​indicate balanced treatment.

With age, the concentration of TSH increases slightly, and the amount of hormones secreted at night - decreases. The normal rhythm of secretion is disturbed when waking up at night.

Decreased TSH is observed in 20% of cases during pregnancy due to an increase in hCG. Transient thyrotoxicosis syndrome is often associated with uncontrolled vomiting in pregnant women. Decreased TSH levels are observed in hydatidiform mole and choriocarcinoma due to a significant increase in hCG levels. In secondary hypothyroidism (central) TSH levels may be normal, but its biological activity is impaired.

Hypothyroidism should be ruled out during hyperprolactinemia.
