General Information

Prolactin (PRL) is a hormone produced by the lactotropic cells of the human pituitary gland, the anterior part of the adenohypophysis. Its release is regulated by the hypothalamus. Dopamine is considered to be the main prolactin inhibitory factor and thus regulates prolactin secretion.

Prolactin is secreted by the anterior pituitary gland and released episodically. Prolactin is found in serum in three different forms; Among them, the monomeric form prevails (80%), which is biologically and immunologically active, 5-20% is the dimeric form ("big" prolactin), which is biologically inactive, and 0,5-5% is the tetrameric form (" "big-big" prolactin), which has low biological activity.

When should we take the test?

In women: Unexplained headaches, visual disturbances, weaning without childbirth and breastfeeding, pain or discomfort during sexual intercourse, hair growth in unusual areas of the body and face, acne, infertility, irregular menstrual cycle.

In men: Unexplained headache, visual disturbances, decreased libido, infertility, erectile dysfunction, abnormal lack of body and facial hair, leakage of milk from the breast.

In girls: Menstrual cycle problems, milk flow from the breast.

In sons: Delayed puberty, underdevelopment of the genitals.

In both sexes, the test is done to rule out problems with the pituitary and hypothalamus.

In patients who have already been diagnosed with a pituitary adenoma, prolactin is tested to monitor the tumor response to treatment.

Possible interpretation of the results

The reason for the increase in prolactin levels may be:

  • Pregnancy;
  • Taking certain medications;
  • Decreased thyroid function;
  • Pituitary adenoma - In general, the concentration of prolactin reflects the size of the tumor. In most patients, after starting treatment, prolactin levels drop significantly and reach normal values in 60-80% of cases.

Reduced prolactin levels:

This is a very rare condition and is found in people with impaired pituitary function. May cause insufficient milk production after childbirth. Low prolactin concentrations are mostly asymptomatic, although studies have shown that patients with low prolactin concentrations may be more susceptible to infections.

Product description

The target organ of prolactin is the mammary gland, where it exhibits lactogenic and galactopoietic effects. During pregnancy, prolactin levels increase as a result of exposure to high levels of estrogen and progesterone. Stimulating action of the mammary gland ensures postpartum lactation.

A high concentration of prolactin interferes with ovarian steroidogenesis, as well as the secretion of pituitary gonadotropes, therefore hyperprolactinemia is one of the most common causes of infertility (in women and men).

TRH and metoclopramide stimulation tests are used to detect latent hyperprolactinemia.

How to prepare for the test?

Because prolactin concentrations vary at different times of the day, it is recommended that the test be performed within 2 hours of waking up in the morning.

Blood should not be taken after a gynecological examination, physical exertion, stressful situations or breast stimulation. It is recommended to abstain from sexual intercourse and alcohol one day before the test.

Birth control pills, antihypertensive drugs and antidepressants can affect the results and they should be stopped as prescribed by your doctor.
