Pemphigus includes a group of autoimmune diseases of the skin and mucous membranes, characterized by damage to the epithelium. Pemphigus includes foliaceus and vulgaris forms.
Pemphigus vulgaris affects both the skin and mucous membranes. In pemphigus foliaceus, only the skin is affected. Other forms of pemphigus develop in lymphomas, various neoplastic syndromes, or as a result of exposure to certain medications.
Both forms of pemphigus are caused by autoantibodies against the keratinocyte surface glycoproteins desmoglein 1 and desmoglein 3. Desmoglein 1 and desmoglein 3 are adhesion molecules. Desmoglein 1-autoantibodies and desmoglein 3-autoantibodies lead to a decrease in their adhesive ability. This is the cause of the blistering skin lesions in pemphigus.
Autoantibodies to desmoglein 1 are found in pemphigus foliaceus, and autoantibodies to desmoglein 3 are found in pemphigus vulgaris.
The diagnostics of pemphigus is based on the detection of autoantibodies, along with histological findings on the skin.
