Testosterone Is an androgen that is synthesized in testicular Leydig cells in men under the influence of LH (melatoninizing hormone), and a small amount is produced by the peripheral conversion of androstenedione. In women, more than 50% of serum testosterone is produced by peripheral conversion of androstenedione, which is synthesized in the adrenal cortex and ovaries, while the rest is secreted in the same organs. Typically, about 60% of circulating testosterone is bound to sex hormone-binding globulin, which is biologically inactive, while the remainder is bound to albumin (testosterone in tissues excretes albumin and spreads rapidly in tissues), and 1-2% circulates as a free fraction.
In men: Evaluation of testicular function in clinical conditions associated with changes in transport proteins (obesity, cirrhosis, thyroid disease).
In women: Hirsutism or virilization.

Both fractions - free and albumin-bound - are found in tissues and are biologically active, while sex hormone-binding globulin i (SHBG) effectively inhibits the action of testosterone.
The free share of testosterone is higher in men, which is due to the inhibitory action of androgens on the synthesis of SHBG.
In women, the proportion of free testosterone is low due to high levels of SHBG synthesis, which is stimulated by estrogens.
One of the most effective methods for assessing biologically active hormone is to quantify the free fraction of testosterone in the serum.
Testosterone levels in men decrease during hypogonadism. Low levels of free testosterone are observed in older men.
If a patient is treated with cortisone or steroid preparations of various origins (natural or synthetic), testosterone levels may not be considered reliable.
In women, free testosterone levels increase in cases of hirsutism, polycystic ovaries, and some other diseases.
