General Information

ClopidogrelTolerance / CYP2C19 enzyme genetic activity test

Clopidogrel genetic testing is a study that examines variants in the gene encoding the CYP2C19 enzyme and determines how effectively the body converts the medication (clopidogrel) into its active antiplatelet form.

CYP2C19The enzyme is a liver microsomal enzyme that is part of the cytochrome P450 enzyme family. This group of enzymes is involved in the metabolism of medications such as clopidogrel, propafenone, omeprazole, sertraline, citalopram, diazepam, and others.

The effectiveness of clopidogrel is significantly dependent on genetic variants of the CYP2C19 enzyme.

Variants in the CYP2C19 gene that reduce enzyme activity are responsible for clopidogrel resistance. Compared with individuals with normal metabolism, carriers of inactivating variants (especially in CYP2C19) have impaired inhibition of platelet aggregation and a higher risk of cardiovascular events (thrombosis, stroke, infarction).

In such patients, clopidogrel dosage adjustment (increase) or replacement with an alternative medication is often required.

Sample:VenousBlood

When should we take the test?

  • Assessment of CYP2C19 enzyme activity before starting clopidogrel treatment
  • Identifying the cause of clopidogrel resistance
  • Identification of patients at high risk of hemorrhagic syndrome

Possible interpretation of the results

The results of the study provide the identification of the genotype encoding the CYP2C19 enzyme, which corresponds to different activities of the enzyme.

The activity of the CYP2C19 enzyme can be increased or decreased by concomitant medications, which directly affects the elimination of drugs that are metabolized by this enzyme.

Drugs that increase the activity of the CYP2C19 enzyme

  • Carbamazepine
  • Prednisone
  • Rifampicin and others

Drugs that reduce CYP2C19 activity

  • Chloramphenicol
  • Cimetidine
  • Felbamate
  • ფლუოქსეტინი
  • Fluvoxamine
  • Indomethacin
  • კეტოკონაზოლი
  • Modafinil
  • Topiramate and others

Reference values

The CYP2C19 enzyme is also involved in the metabolism of other drugs (statins, antidepressants, proton pump inhibitors).

The Clopidogrel test is a genetic test that checks for changes in the CYP2C19 gene. This gene codes for the cytochrome P450 2C19 enzyme, which is responsible for the metabolism of clopidogrel and other drugs. The test results help your doctor plan personalized treatment.
