The 11-deoxycortisol test is used to diagnose congenital adrenal hyperplasia. 11-deoxycortisol is a precursor to cortisol. Its level increases significantly with a significant increase in the concentration of adrenocorticotropic hormone (for example, in Cushing's syndrome, corticotropin-synthesizing tumors, etc.) or in the case of 11-beta-hydroxylase deficiency.

Elevated serum 11-deoxycortisol in suspected congenital adrenal hyperplasia indicates a possible 11-beta-hydroxylase (enzyme) deficiency. In a number of cases, an increase in serum 11-deoxycortisol concentration is observed only in response to stimulation with adrenocorticotropic hormone.
