General Information

Human epididymal protein 4 (HE4) is synthesized in ovarian epithelial cells. That is why it is a specific marker of ovarian epithelial cell tumors and is used for differential diagnostics and also for monitoring diagnosed epithelial cell tumors. The test determines the concentration of EH4 in the blood serum.

There are several subtypes of epithelial cells: serous, endometrial, mucinous, and clear cell. The most common serous epithelial cell tumors.

According to studies, the increase of EH4 is more typical for ovarian serous and endometrial epithelial cell tumors, less for clear cell tumors. And, in the case of mucous tumors, its concentration is almost not increased.

Human epididymal protein 4 (HE4), together with CA-125 carcinoembryonic antigen, is used to monitor ovarian epididymal tumors. The combination of these two studies allows for better monitoring.

The use of HE4 for screening in asymptomatic women is futile because it is a non-specific test. A small amount of HE4 is also synthesized by other cells, so a moderate increase in its concentration is also possible in some non-cancerous diseases.

HE4 and CA125 are part of the ovarian malignancy risk assessment algorithm (ROMA-index), which is a reliable marker for the differential diagnostics of small pelvic bulky processes.

If before the start of treatment, the concentration of the markers was significantly increased, it is recommended to control the indicators in the dynamics to evaluate the effectiveness of the treatment.

If the level of HE4 was normal before treatment, its control in dynamics is uninformative.

Increased indicators before treatment, decrease in the background of treatment - indicates positive dynamics, i.e. Treatment has results.

If after treatment, the increased concentration of HE4 does not change, this indicates the ineffectiveness of the treatment.

After surgical treatment of bulky tumors of the small pelvis, an increase in the ROMA-index probably indicates a malignant process.

Reference values

 

In different laboratories, EH4, CA125, ROMA-index studies may be conducted with different methods, therefore, comparing the data of different laboratories in order to observe the dynamics is not recommended. It is always better to compare data from studies conducted in the same laboratory in terms of dynamics.
